Dorsal rays XXVII, 24-51 elements, the first dorsal spine elongate; anal rays II, 38; pectoral rays 13; body elongate, the depth 12.3 in standard length; posterior margin of preopercle with serrations and short spines; first infraorbital with strong spines on ventral margin; a raised ridge on frontals; widely separated nasal bones with prominent raised ridges and a single row of projecting bony knobs; no cirrus on anterior nostril; one pair of flattened, unbranched eye cirri; snout pointed; mouth oblique.

A small orange blotch on cheek behind upper jaw; white spots or blotches on head, body and dorsal fin; opercle, pectoral-fin base, and basal portion of pectoral-fin rays each with a single large white blotch; entire length of body with alternating white and dark saddles.

Size: to at least 5.4 cm.

Habitat: sand/cobble bottoms.

Depth: 17 m on a sandy bottom.

Known only from a single specimen collected at Isla Rey, Islas Perlas, Panama.
