Body moderate depth, compressed; head round; with fleshy crest along top of head and nape; mouth at front, oblique; head without lateral line pores; papillae around mouth, on cheeks and operculum and over eye; front nostril tubular, rear nostril a hole with raised front rim; tongue rounded to blunt, not bilobed; 1st dorsal fin with VI spines, 2nd & 3rd spines filamentous in male, 18-20 (usually 19) dorsal rays; 15-16 anal rays; 20-23 (usually 21-22) pectoral rays; pelvics fused into disc; scales large, rough; head, nape, chest and base of pectoral scaleless; ~43 lateral scale rows; tail short, round, with 17 segmented rays.
Red; 4-7 thin blue, dark-margined bars on body and 3 on head; dorsal, tail and anal fins orange, paler distally; pectorals and pelvics blue-grey; 5 black dots along underside of tail base.
Size: 2.6 cm.
Habitat: rocky bottoms.
Depth: 30-90 m.
Cocos Island.
