Body elongate (depth 12.5% of standard length); body and head flattened; eye large (30% of head length), narrow between eyes; head without pores; papillae on most parts of head and nape; mouth short (36% of head), lower jaw with two rear canines; tongue tip deeply indented; VII dorsal spines, 12 dorsal rays; 11 anal rays, pectoral short, 19 rays; pelvic fins short, completely separate, rays branched; scales on body behind a line under end of 1st dorsal, large rough scales on base of tail fin, head and front of body scaleless.
Color Pattern of ~5-6 narrow bars on body and lines radiating down from eye.
Size: 2.2 cm.
Habitat: probably rubble and sand.
Depth: 20-40 m.
Known only from a single specimen collected at the Galapagos Islands.
